My supporting immunization documentation does not have my date of birth (DOB) printed on it, what should I do?

Any supporting immunization documents must have two identifiers. Your full name must be one of the identifiers.

Students should write their date of birth (DOB) and Mason G# on the documentation to submit it. Preferred DOB format is MM/DD/YYYY.
