Nurse Case Management

Student Health Services nurse case managers make navigating healthcare easier for students with chronic or short-term health conditions.

Nurse case management supports students by helping them connect with needed medical care. It can also help you better understand your health concerns. Case managers support students with concerns like:

  • elevated cholesterol
  • disordered eating
  • healing after an operation
  • orthopedic injuries
  • and many other concerns.

How can case management help?

  • Give information and answer questions about your diagnosis and treatment plan
  • Identify health needs
  • Work with you to set personal health goals
  • Schedule appointments with specialists and off-campus providers
  • Connect you with on-campus and off-campus services and resources
  • Help you understand your health insurance plan
  • Help find in-network providers
  • Identify barriers to care and discuss options

How to connect with case management

For students currently seeing a healthcare provider at Student Health, ask your provider for a case management referral. A nurse will contact you to schedule a case management appointment. These appointments can be in-person at the Fairfax Clinic or a scheduled phone call.

Students must first be an established patient at Student Health Services before meeting with a case manager. If you are a new patient, schedule an appointment with a Student Health Services provider. When you schedule or at your appointment, let us know that you are interested in working with a nurse case manager.

For general questions, call 703-993-2831 (select option 1) during operating hours and leave a message for a nurse case manager. They will return your call within one business day.
