Transcription Service

The Immunization Office offers transcription service for immunization (shot) records for a fee.

How to request transcription service

  1. Fill out student sections (page 1 and 2) of the George Mason Certificate of Immunization Form.
  2. Sign the transcription consent on the bottom of page 1 of the form. 
  3. Upload the Certificate of Immunization, your shot records and immunization documents to the Patient Portal. Follow instructions on the portal “Upload” page. Documentation must be in English, have patient name and date of birth. 
What if I did not sign the transcription consent on the Certificate of Immunization?

If you have already submitted your immunization records, then you must email the Immunization Office (immunize@gmu.edu) to request transcription service.  The email is only to request transcription, no attachments accepted. Upload to the Patient Portal.

The email must include: 

  • Your full name
  • Mason G# (student id #)
  • The phrase “Request for transcription service”
  • Written acknowledgement that you agree to pay the transcription service fee.

Do not prepay the transcription service fee. Do not pay fee until service is completed and you receive a confirmation message.
