Regardless of age, all incoming students are required to complete the tuberculosis screening (page 2) portion of the Immunization Record Form.
The Tuberculosis (TB) Screening Questionnaire is required for all incoming students as defined by the Centers for Disease Control and Prevention (CDC) and the Virginia Department of Health. The Tuberculosis (TB) Screening Questionnaire must be completed and submitted within the last 6 months.
If a student answers ‘yes’ to any question in the TB Screening section, a Tuberculosis test (TB test) or blood test must be performed within 6 months of beginning classes. The Immunization Office must have the following documentation:
- Date placed
- Date read (48-72 hours after placement)
- The results in # (mm) form
If the TB test is positive or the student has a history of positive TB, the Immunization Office must have a copy of the test results along with a copy of the most recent chest x-ray report. If a student received treatment for TB or LTBI the student must provide documentation of medication, date started, and date completed.

All new incoming summer/fall students must submit immunization records by the deadline. Records turned in after the deadline or records that are incomplete after the deadline will be charged a late fee and a hold will be placed on the student's account.

The Immunization Office will be unable to take phone calls during this time. Please email (response within 48-72 hours) or stop by the office regarding immunizations. Phone lines will reopen October 9, 2017.
Past Due Immunization Records
If you have a hold and late fee, you must complete the immunizations and pay the late fee to Student Health Services to have your hold removed. The hold will be released usually by the end of that business day, if after 4:30 pm it will be processed the next business day.
Note: Do not include social security numbers on your immunization records.
