All students born after December 31, 1956 must provide proof of compliance with immunization requirements mandated by the Commonwealth of Virginia and George Mason University.
Students are strongly encouraged to meet immunization requirements at home and return the necessary forms as soon as possible. There are five immunization requirements (listed below).
You must provide proof of immunization for each requirement. Accepted documentation is listed in the A, B, or C columns in the chart below.

|Tuberculosis||The Tuberculosis (TB) Screening Questionnaire is required for all students (Part 4 of the Immunization Record Form). The TB screening questionnaire must be completed and submitted within the last 6 months. Defined by the Centers for Disease Control and Prevention (CDC) and the Virginia Department of Health.|
|Tetanus, Diphtheria||Tetanus/Diphtheria vaccine within the last 10 years||Tetanus Diphtheria and Pertussis (Tdap) within the last 10 years|
|Measles, Mumps, Rubella
||Two (2) doses of the MMR (Measles, Mumps, Rubella) vaccine administered after your first birthday AND after 1967.||Titer report indicating positive immunity|

If a student is lacking the required immunizations, s/he must either provide proof of receiving all necessary vaccines or receive the necessary vaccines.

Immunization records due by March 1st.
All newly admitted or newly re-admitted spring students must submit immunization records by the deadline. Records turned in after the deadline or records that are incomplete after the deadline will be charged a late fee and a hold will be placed on the student's account.
Past Due Immunization Records
If you have a hold and late fee, you must complete the immunizations and pay the late fee to Student Health Services to have your hold removed. The hold will be released usually by the end of that business day, if after 4:30 pm it will be processed the next business day.
Note: Do not include social security numbers on your immunization records.
