** If you need to drop off your records after-hours there is a drop box next to the immunization office in SUB I.**


 

Immunization Requirements:

 

The Immunization Record Form must be filled out with the following five requirements:


1. Hepatitis B

Documentation of 3 doses of Hepatitis B or Twinrix

OR

A titer report (blood test) showing positive immunity

OR

A signed waiver (located in the list of immunization forms)

 

2. Tetanus and Diphtheria

Tetanus/Diphtheria vaccine within the last 10 years

OR

Tetanus Diphtheria and Pertussis (Tdap) within the last 10 years


3. Measles, Mumps, Rubella (MMR)

Two (2) doses of the MMR (Measles, Mumps, Rubella) vaccine administered after your first birthday AND after 1967.


OR

Titer report (blood test) indicating positive immunity



4. Meningococcal Vaccine

Must receive vaccine between the ages of 16-21.*

OR

A signed waiver (located in the list of immunization forms)

*If you are 22 years of age or older you are not required to have the meningococcal vaccine.


5. Tuberculosis Screening

The Tuberculosis (TB) Screening Questionnaire is required for all students as defined by the Centers for Disease Control and Prevention (CDC) and the Virginia Department of Health. The TB screening questionnaire must be completed and submitted within the last 6 months (located in the list of immunization forms).

Deadlines:

  • October 1st for incoming Fall Students

  • March 1st for incoming Spring Students

Late Fees and Holds:

  • If you are making a payment online, allow 24 – 48 business hours for your payment to reflect in our system. Fees will be processed Monday thru Friday only! Payments will not be processed on Holidays, weekends, or day Mason is closed.
  • Immunizations holds will be removed once records are complete and payment is reflected.

Immunizations available by appointment:

All required vaccines are available at Student Health Services for a fee.

To schedule an appointment, please call 703-993-2135.

The immunization staff will schedule appointments for students only after records have been reviewed and missing vaccines have been determined.

After you submit your immunization records to the Fairfax Campus Immunization Office, please contact the Immunization Office to find out if any vaccinations are required, and schedule your immunization appointment, if necessary.

Immunization Record Completion


As a student registered for classes at George Mason University, you must be fully compliant with Mason’s and the Commonwealth of Virginia's immunization requirements. If you are lacking requisite immunizations, you must either provide proof of receiving all necessary vaccines, or take these vaccinations from Mason’s Student Health Clinic, your health care provider, or the health department.


Students are strongly encouraged to meet these requirements at home and return the completed form as soon as possible. The forms can be faxed or mailed to the Immunization Office.

Parents: please remember that if you send the forms to school with your student, there is a possibility they will be misplaced, forgotten, or lost. The transition from living at home to living on campus can be a hectic experience.

Immunizations can be obtained through local health departments in Virginia or through private health care providers. Students may also obtain all needed immunizations through Student Health Services. Vaccines are available on all George Mason University campuses by appointment only. Minimal fees
are charged for all vaccines given.

Immunization Office will be closed for staff development


Past Due Immunization Records

All late records will be assessed a $25 late fee and a hold will be placed on your Patriot Web account.

If you have a hold and late fee, you must complete the immunizations and pay the $25 late fee to Student Health Services to have your hold removed.

The hold will be released by the end of that business day, if after 4:30 pm it will be processed the next business day.
