Immunization Records are due OCTOBER 1, 2009

for students starting in the FALL 2009 Semester.   

ALL late records will be assessed a $25 LATE FEE.

Immunization Record Completion

Each semester Student Health Service sends incoming students an Immunization Record outlining the requirements for immunizations. Compliance with and completion of this form is a Commonwealth of Virginia and George Mason University requirement for all students. Students will not be allowed to register for a second semester without full compliance.

Students are strongly encouraged to meet these requirements at home and return the completed form as soon as possible. The forms can be faxed or mailed to the Immunization Office.

Parents: please remember that if you send the forms to school with your student, there is a possibility they will be misplaced, forgotten, or lost. The transition from living at home to living on campus can be a hectic experience.

Immunizations can be obtained through local health departments in Virginia or through private healthcare providers. Students may also obtain all needed immunizations through Student Health Services. A Immunization Walk-In Clinic is offered at the Fairfax campus. Vaccines are available on all George Mason University campuses. Minimal fees
are charged for all vaccines given.

Note: If you were born before 1957, completion of the immunization form is NOT required. No further action is needed.

Immunization Requirements

1. Hepatitis B must receive 3 doses of vaccine

OR

provide titer report indicating positive immunity or sign waiver


2. Tetanus and Diptheria: Last booster within 10 years.

OR
Tetanus Diphtheria and Pertussis (Tdap) for the decennial booster

3. Two (2) doses of the MMR (Measles, Mumps, Rubella) vaccine administered after your first birthday AND after 1967.

OR
Two (2) doses of the following vaccine: Measles vaccines, mumps and rubella. All must have been administered after your first birthday AND after 1967.
OR

Titer report indicating MMR immunity is acceptable in place of vaccine documentation.

4. Meningococcal Vaccine: must receive vaccine or sign
waiver


5. Tuberculosis Screening - Tuberculosis (TB) screening is required for all students as defined by the Centers for Disease Control and Prevention (CDC) and the Virginia Department of Health. TB screening, shall be completed within the last 12 months. Please see the Tuberculosis Screening Questionnaire.

If you would like a copy of your immunization records please fill out the following release form and fax to the Immunization Office. Your request will be processed within 1-2 business days.
